Leon Knight scored twice as the Swans booked their place in the League One Play-Off Final following victory over Brentford at Griffin Park.
The two teams were all-square going into this second leg following a 1-1 draw at the Liberty Stadium a week previously.
But the visitors almost fell behind inside the first minute as Jamie Smith's effort was well saved by Willy Gueret.
However, the Swans broke the deadlock after eight minutes as Knight coolly chipped the ball over the onrushing Stuart Nelson and into the net.
The striker completed his brace seven minutes later as Leon Britton teed him up before the former Chelsea man smashed the ball past Nelson.
The Bees tried to force their way back into the game as Jay Tabb shot wide, while Alex Rhodes saw an effort saved by Gueret.
But the Swans held firm throughout to seal a place in the play-off final at the Millennium Stadium where they would face Barnsley for a place in the Championship.
