A single goal from Sean McCarthy handed the Swans a narrow lead going into the second leg of their Division Four Play-Off semi-final against Rotherham United at the Vetch.
The striker was again on the scoresheet in the return game as the Swans booked their place in the two-legged final against Torquay, which they won 5-4 on aggregate.
It meant the Swans won promotion to Division Three along with champions Wolverhampton Wanderers, runners-up Cardiff City and Bolton Wanderers. Newport County, meanwhile, suffered a second consecutive relegation after they finished bottom.
